Position Summary
CFTSS (Children and Family Treatment and Support Services) and HCBS (Home and Community-Based Services) offer opportunities to better meet the behavioral health needs at earlier junctures in a child/youth’s life to prevent the onset or progression of behavioral health conditions. CFTSS is a set of services designed to help youth and families to gain the necessary tools to be successful in their homes and communities through goal-directed support and solution-based interventions. HCBS is designed to allow children/youth to participate in a vast array of developmentally and culturally appropriate mental health services.
Responsibilities:
- Provide face-to-face intervention with child/youth, family/caregiver, or other collateral supports;
- Provides individual, group, and family counseling both on and off-site;
- Provides counseling around crisis avoidance and crisis management
- Coordinates with other providers involved in the individual’s treatment;
- Maintains records in an accurate and timely manner in accordance with appropriate regulations;
- Creates treatment plan in order to describe the condition and services needed for care, detailing scope/practices to be provided, expected outcome, and expected frequency and duration of the treatment
- Provides psychoeducation and “hands-on” interventions to child/youth and family members in order to promote skill development;
- Attend required supervision, meetings, and training;
- Continuous monitoring that family choice and voice are present in service delivery;
- Maintains information on community resources
General Responsibilities:
- Attends workshops, conferences, and staff development training seminars on current practices as directed by the Chief Executive Officer or Program Director to acquire continuing education in the appropriate field; implements what has been learned;
- Maintains cooperative relationships with referent and community resources
- Undertakes related duties as assigned;
Administrative Relationships:
The CFTSS Clinician reports directly to the Program Director or designee and collaborates with other staff.
Qualifications:
A master’s degree in social work, psychology, or related human services, plus one year of applicable experience or one who has been certified in an Evidenced Based Practice (in lieu of one year experience requirement). Licensed Master Social Worker, Clinical Social Worker, Licensed Mental Health Counselor, or a Mental Health Counselor with a Limited Permit required. Must have basic computer literacy and excellent verbal and written skills. A valid driver’s license is required.
Reasoning Ability:
Ability to apply common sense and understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problem-solving several concrete variables in standardized situations.
