PROS Employment Specialist (Hybrid)

Position Summary

The Employment Specialist is responsible for promoting Health Home and Care Management services, and educating the surrounding resources, potential applicants and the community about these services.  In promoting such services, the Employment Specialist will spend considerable amounts of time in the community, with the expectation of generating appropriate referrals to the above programs and ensuring that eligible applicants have access to these services.  Additionally, this position will provide intake assessments and provide services to engage Health Home clients identified as Outreach.  Lastly, the Outreach Specialist may carry a small caseload of clients, coordinating services and supports for individuals who meet the Care Management eligibility criteria in order to help successful integration into and maintenance in the community.

Responsibilities: 

  • Provides job development through creating partnerships with local employers.
  • Provides job coaching and other interventions that are targeted to helping individuals succeed in a specific job-related task;
  • Provides direct support in helping an individual find and procure a job, including resume writing, completing applications, scheduling, and participating in interviews.
  • Intervenes with an employer to resolve an issue regarding the individual or the workplace.
  • Assists with benefits and financial management.
  • Assists with connecting with outside benefit navigators.
  • Assists with benefits paperwork or applications.
  • Attends NYESS training and implements all NYESS documentation requirements.
  • Completes IPS annual fidelity self-assessments.
  • 50% of the time will be spent in community-based service delivery.
  • Provides individual and group counseling and case management on-site, in the field, or the home.
  • Provides vocational readiness activities and vocational assessments for individuals.
  • Participates in interdisciplinary team meetings and conferences.
  • Refers the applicants to ancillary services.
  • Coordinates and acts as a liaison with other community service providers.
  • Accompanies clients to offsite activities.
  • Maintains records in an accurate and timely manner per regulations.
  • Collects and synthesizes data for use in problem-solving to assess and address a client’s needs.
  • Understands and disseminates information on community resources; particularly public benefits and entitlements.
  • Assists with program and agency crises.

Administrative:

  • Submission of timesheets in a timely manner.
  • Communicates any possible schedule changes as soon as possible and always considers client needs and agency coverage when asking for flexible scheduling.
  • Maintains a 90% or higher customer satisfaction rating.
  • Maintains all documentation in OMH portals as required (CAIRS, NYESS) within 5 days of the due date
  • Maintains 100% UM/UR compliance with charts by entering information into the chart correctly 99% of the time.

Administrative Relationships:

Position reports to PROS Program Supervisor or Director.

Physical Demands:

  • The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
  • The employee must be able to sit and remain in a stationary position for durations of time.
  • The person in this position needs to constantly operate a computer and other office productivity machinery.
  • The person in this position occasionally ascends/descends a ladder.
  • The employee must have the ability to communicate information and ideas so others will understand.
  • The employee must be able to exchange accurate information in these situations. The employee must have the ability to observe details at close range (within a few feet of on server).
  • The employee must be able to lift/or move up to 5 lb. and occasionally lift/or move up to 10 lb.

Qualifications:

Education: Must be at least 18 years of age and have a high school diploma or equivalent with 1-3 years of relevant experience in a behavioral health-supported employment setting.

  • An insurable driver’s license is required, and the use of own car may be needed.
