Licensed Clinician LSCW/LMHC Satellite Clinics School District

Position Summary

The Social Worker shall use psychotherapeutic skills in the provision of clinical and case management services to people with mental illness and other co-occurring disorders and shall establish and maintain contact with professionals and family advocates in the community for the purpose of maximizing services for students.

Responsibilities: 

  • Working at (Putnam Valley High School/ Mahopac and Brewster school district) with the students to help address a range of mental health concerns.
  • Provides the intake and screening of new applicants and participates in clinical review/case disposition meetings where treatment recommendations are formulated.
  • Participate in interdisciplinary team conferences within the agency when indicated.
  • Provides crisis intervention and makes recommendations for treatment and referral.
  • Provides consultation and advocacy for individuals in inpatient, residential, and independent living situations, including on-site visits and discharge meetings.
  • Maintains records in an accurate and timely manner in accordance with appropriate regulations.
  • Collects and synthesizes data to assess an individual’s needs.
  • Maintains information on community resources.
  • Determines the need for other treatment modalities and refers applicants to ancillary services.
  • Meets productivity standards.
  • Assists with program and agency crises.
  • Flexes to adjust to the needs of the individual and other family members, including community visits when requested.
  • Facilitates hospitalization and collaborates with inpatient staff regarding treatment and discharge planning.

Administrative:

  • Submission of timesheets in a timely manner.
  • Communicates any possible schedule changes as soon as possible and always considers client needs and agency coverage when asking for flexible scheduling.
  • Maintains high client satisfaction as evidenced by no more than 1 request for client transfer every 3 months for non-technical reasons.
  • Maintains between an 85% to 95% appointment-kept rate with a no-show rate remaining below 10%.
  • Maintains UM/UR compliance with charts by entering information into chart per program standards:

-Notes done and signed daily.

-Treatment plans completed and routed before they are due.

-Closings done within 90 days of last session.

Physical Demands: 

  • The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
  • The employee must be able to sit and remain in a stationary position for an hour or more.
  • The person in this position needs to constantly operate a computer and other office productivity machinery.
  • The employee must have the ability to communicate information and ideas so others will understand.
  • The employee must be able to exchange accurate information in these situations. The employee must be able to observe details within 3-6 feet.
  • The employee must be able to lift/or move up to 5 lb. and occasionally lift/or move up to 10 lb.

Administrative Relationships:

Position reports to Clinic Supervisor.

Qualifications:

  • Education MA or MSW in Counseling or Social Work.
  • Experience – one year of experience preferred.
  • Independent Licensure Required
  • (LCSW or LMHC)
  • Other – Background check clearance

Preference is given to candidates who are bilingual in Spanish
