Program Coordinator- Care Management-Outpatient Behavioral Health

Position Summary

The Program Coordinator of Care Management is responsible for the overall running of the Care Management Program and interfacing with the area Health Homes. The coordinator supervises the work of the program staff, assists the Program Director with program development, quality assurance, and utilization review, as well as coordinates services and supports for a reduced caseload of individuals with a primary diagnosis of severe mental illness and/or other qualifying disorders in order to help successful integration into and maintenance in the community.

Responsibilities:

  • Ensures all statistical and billing reports and forms are accurately prepared and sent to billing office and Health Home within time frames specified.
  • Acts as conduit between the Health Home and PFCS; becomes proficient in use of Health Home EHR
  • Supervises case disposition including assessment, admission, referral, and discharge of clients into services that provide optimal support and treatment.
  • Assures that all requisite documentation/charting is completed in an accurate and timely manner in accordance with appropriate regulations.
  • Provides guidance and support to all program staff.
  • Provides direct and general supervision to program staff and monitors and addresses productivity and performance.
  • Collaborates with other community-based programs in order to coordinate services for eligible individuals and families.
  • Markets and represents the program in the community via on-site presentations and telephone outreach in order to elicit appropriate referrals.
  • Assists Program Director in preparing administrative, fiscal, statistical, and clinical reports.
  • In conjunction with Program Director, provides staff with updated information on agency-wide activities and other related services in the community.
  • Meets productivity standards.
  • Assesses client’s success and satisfaction in the environments of living, learning, working and socialization.
  • Implements and coordinates the service plan in conjunction with the client and involved supports.
  • Advocates for and assists clients in gaining access to entitlements and benefits, health, and human services, including preparation and coordination of applications.
  • Assumes full responsibility for caseload of individuals including all charting and paperwork responsibilities associated with cases.
  • Responsible for crisis coverage of caseload when indicated.
  • Provides outreach in the community via home visits to engage clients and link to services.
  • Identifies and advocates for improvements in the services that our clients receive.
  • Participates in care management and other inter-disciplinary team meetings for case reviews, exchange of information and supervision.
  • Compiles, reports, and maintains records in an accurate and timely manner in accordance with appropriate requirements.
  • In addition, the below responsibilities may be required of the position:
  • Assists with agency and after-hours crisis coverage, intervention, and assessment.

General responsibilities:

  • Attends workshops, conferences and staff development training seminars on current practices as directed by the Executive Director or supervisor to acquire continuing education in the appropriate field; implements what has been learned.
  • Maintains cooperative relationships with all referents and community resources
  • Undertakes related duties as assigned.

Administrative Relationships:

The Program Coordinator reports directly to the Program Director or designee and collaborates with other staff.

Qualifications:

Bachelor’s degree in human services or related field required; master’s degree in such is required. Candidate must be licensed or with a limited permit. Must have minimally two years’ experience working in the mental health field; some care management and supervisory experience is preferred. Must have excellent computer literacy and excellent verbal and written skills. Valid driver’s license and use of own car required. Must have the ability to travel in the community and access clients in their own homes.

Reasoning Ability:

Ability to apply common sense and understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problem solving several concrete variables in standardized situations.

Salary: $50,000- $55,000 per year
